You can apply in one of two ways:
- You may print the application in .pdf format
(Adobe Acrobat Reader required), complete it,
and mail it to our office at the following address:

- Come to the LCO Vocational Rehabilitation office
location to apply in person.
Our office hours are 8:00 a.m. to 4:30 p.m.
Monday through Friday with the exception of holidays and other closures.
You may wish to call in advance at (715) 638-5161.
After you submit an application, we may request additional documents to determine
your eligibility, such as your tribal ID and medical records on disability.
